Neo-adjuvant Chemo and immunotherapy with durvalumab (MEDI4736) and tremelimumab (MEDI1123) In The pre-operAtive Treatment of locally advanced cholangIOcarciNoma: an exploratory and translational study.

This clinical trial is testing a new way to treat locally advanced cholangiocarcinoma, a type of cancer that starts in the bile ducts. Doctors are combining standard chemotherapy drugs (gemcitabine and cisplatin) with two immunotherapy medicines (durvalumab and tremelimumab). Immunotherapy works by helping your body's own immune system find and attack cancer cells. Participants receive this treatment combination before undergoing surgery to remove their tumour. Giving treatment before surgery is known as neoadjuvant therapy. The main goal of the study is to see whether this drug combination can shrink the cancer enough to make surgery more effective and stop the disease from returning within 12 months. Researchers will also closely monitor safety, side effects, and how long patients remain well after treatment.

Vad handlar denna studie om?

Bile duct cancer, also called cholangiocarcinoma, is an uncommon cancer that affects the tubes connecting the liver and gallbladder to the small bowel. When the cancer is 'locally advanced', it has grown into nearby tissues, which can make it very difficult for surgeons to remove safely. Finding better ways to shrink these tumours before an operation is an important area of cancer research.

In this study, researchers are testing a mix of treatments given before surgery. Patients receive two standard chemotherapy drugs alongside two newer medicines called durvalumab and tremelimumab. These newer drugs are types of immunotherapy. Instead of attacking cancer cells directly, they help your own immune system recognise and fight the cancer.

By giving chemotherapy and immunotherapy together before surgery, doctors hope to shrink the tumour and clear any microscopic cancer cells nearby. The trial will measure how many patients remain cancer-free a year after their operation. The results could help doctors understand if this combination offers a better, more lasting option for people facing this challenging illness.

Viktiga punkter

  • Tests a mix of chemotherapy and immunotherapy before surgery for bile duct cancer.
  • Aims to shrink tumours to improve surgical success and lower the chance of cancer returning.
  • Open to adults aged 18 and over with locally advanced cholangiocarcinoma.
  • Involves regular hospital visits for drip infusions, blood tests, and scans.
  • Participation is entirely voluntary and you can withdraw at any time.

Vem kan vara berättigad?

This trial is open to adults aged 18 and over who have been diagnosed with locally advanced bile duct cancer. Both men and women can take part. To join, a person's cancer must be in a stage where treatment before surgery is considered a safe and sensible next step by their medical team.

People will need to have a general health check to make sure their liver, kidneys, and heart are strong enough to handle chemotherapy and immunotherapy. Because these medicines are powerful, patients who are very frail, who have certain active autoimmune conditions, or who are pregnant may not be able to join.

Your cancer specialist will review your scans, medical history, and blood tests to confirm whether this trial is a suitable and safe option for you.

  1. I am aged 18 or older.
  2. I have been diagnosed with locally advanced bile duct cancer (cholangiocarcinoma).
  3. My medical team is considering surgery as part of my care plan.
  4. I am well enough to attend hospital visits for regular intravenous treatment.
  5. I do not have severe medical conditions that prevent me from receiving chemotherapy.

Vad innebär deltagande?

If you take part, you will receive treatment through a drip into a vein (an intravenous infusion) at a hospital clinic before your planned operation. The treatment includes standard chemotherapy medicines (gemcitabine and cisplatin) combined with immunotherapy drugs (durvalumab and tremelimumab). You will need to visit the hospital regularly for these infusions and for routine health checks.

During your visits, the healthcare team will run blood tests, check your vital signs, and carry out imaging scans (such as CT or MRI scans) to see how the cancer is responding. After finishing the course of medication, you will have your surgery as planned.

Following surgery, your care team will monitor your recovery closely. You will have regular follow-up appointments and scans over the course of at least 12 months to track your health and check that the cancer has not returned.

Potentiella risker och fördelar

Taking part may help shrink your tumour before surgery, which could improve the chances of a successful operation and help keep the cancer away for longer. However, there is no guarantee that the treatment will work for everyone. Chemotherapy and immunotherapy can cause side effects such as tiredness, sickness, an upset stomach, increased risk of infections, or inflammation in healthy organs. Your medical team will monitor you very closely and treat any side effects promptly. Taking part is completely voluntary, and you are free to leave the trial at any point without it affecting your standard NHS care.

Vanliga frågor

Vad är immunterapi?

Immunotherapy is a type of cancer treatment that boosts your body's natural defences, helping your immune system spot and destroy cancer cells.

Why is treatment given before surgery?

Giving medicines before an operation aims to shrink the tumour, making it easier and safer for the surgeon to remove completely.

Do I have to pay for the trial medicines?

No. In UK clinical trials, the study medications and extra tests are provided free of charge.

Kan jag lämna studien om jag ändrar mig?

Yes. You can leave the trial at any time. Your doctors will continue to give you the best available standard treatment.

Will I still have surgery?

Yes, having surgery to remove the tumour is a planned and central part of this study, provided you remain well enough for the operation.
